E. Patient Confidentiality Consent for Transfer Arrangements between Centres
At Sussex Sperm Bank (SxSB) we take your confidentiality very seriously. We require your permission to discuss the transfer of donor sperm samples to your treatment centre. We will only disclose any identifying personal data under the terms below and not for any other purpose, in line with the General Data Protection Regulation.
I/We understand and agree this will include the following points involving our personal data:
- We must have third party agreements with all centres, which specify the terms and conditions of the transfer process between fertility centres. Personal data from you and the chosen donor will be transferred between centres and used for checking purposes and to identify the correct gametes for your treatment. In lieu of a third party agreement, SxSB may be able to perform a Risk Assessment to allow for the distribution of donor specimens to the clinic. However, please be aware this will not be satisfactory in all settings.
- We may use your name and DOB to ensure that we are in agreement with the centre when identifying the donor sample recipient.
- Discussions between centres are to confirm arrangements of couriers, dates and timings with lab staff availability, which may be by phone and/or email. These discussions are made between licensed clinic staff only.
- The laboratory details of the donor samples purchased from SxSB which may impact on your treatment cycle.
- Personal data, including financial records, at SxSB will be kept in line with HFEA data requirements.
- Personal data is used to discuss treatment outcomes with your treatment centre.
- You have a right to know what personal data we hold about you at any time. All requests must be in writing.
By signing SxSB Form J, I/We give permission for staff at Sussex Sperm Bank (SxSB) to discuss the arrangements for the transfer of donor sperm from SxSB to the receiving treatment centre, as well as treatment plans and outcomes regarding your treatment(s).
